This checklist is to help you complete the requirements for the admission packet for the College of Education Cohort Program. If you have questions regarding the items on this list, please contact your GCSU pre-education advisor, Mrs. Sharon Spink (478) 445-6428 . If you do not currently attend GCSU or if you have questions regarding your choice of program, please contact the Dean’s office at (478) 445-2514.

The following items complete the application packet for the cohort program. All documents in the application packet should be returned by the priority date of the third Friday in January--1/15/2010.

_____ 1. Information Sheet for Application

_____ 2. Official copy of transcript(s) or tranguide (if GCSU student)
               including the current fall semester

_____ 3. GACE Basic Assessment scores or exemption evidence. This evidence must be on the official GCSU Banner site and can only be put there
               from an official high school transcript with SAT/ACT scores or official copies from the testing service.  College transcripts from other schools    
               are not official.

_____ 4. Evidence of passing or exemption of Regents' Reading and Writing  Exams.

_____ 5. Copy of your GCSU admission letter if you are a transfer student

_____ 6. Consent Form for Background Check

_____ 7. Three (3) Professional References using this format

_____ 8. Applicant Questionnaire

_____ 9. Record of Experience with Young Children (Early Childhood applicants only)
